Senior Coroner, Crispin Butler said a number of substances were found at the scene though police found that nothing was 'untoward' and assumed he had suffered an overdose or a medical episode.

A subsequent post mortem identified a number of substances in Simon's system including an amount of morphine that is thought to have arisen from the use of heroin.

Mr Butler said their was a 'toxic level' of morphine, 'within the range encountered in fatalities' as well as cocaine in his blood.

A medical cause of death was given as heroin toxicity contributed to by a number of other substances.

The coroner concluded that Simon's death was drug related.

He added that the 47-year-old, who lived in Victoria Street, High Wycombe at the time, hadn't intended to harm himself or end his life.

Mr Butler added: "An entirely unintended and a very sad end.

"[Emergency services] seem to have done everything they can to try and reverse the affects of substances.
